A baby’s teeth begin coming in just a few months after birth—first one or two in the front, and then gradually the rest of them over the next couple of years. We often refer to these primary teeth as deciduous—just like trees of the same description that shed their leaves, a child’s primary teeth will all be gone by around puberty.
It’s easy to think of them as “minor league,” while permanent teeth are the real superstars. But although they don’t last long, primary teeth play a big role in a person’s dental health well into their adult years.
Primary teeth serve two needs for a child: enabling them to eat, speak and smile in the present; but more importantly, helping to guide the developing permanent teeth to erupt properly in the future. Without them, permanent teeth can come in misaligned, affecting dental function and appearance and increasing future treatment costs.
That’s why we consider protecting primary teeth from decay a necessity for the sake of future dental health. Decay poses a real threat for children, especially an aggressive form known as early childhood caries (ECC). ECC can quickly decimate primary teeth because of their thinner enamel.
There are ways you can help reduce the chances of ECC in your child’s teeth. Don’t allow them to drink throughout the day or to go to sleep at night with a bottle or “Sippy” cup filled with milk, formula, or even juice. These liquids can contain sugars and acids that erode enamel and accelerate decay. You should also avoid sharing eating utensils with a baby or even kissing them on the mouth to avoid the transfer of disease-causing bacteria.
And even before teeth appear, start cleaning their gums with a clean, wet cloth right after feeding. After teeth appear, begin brushing and flossing to reduce plaque, the main trigger for tooth decay. And you should also begin regular dental visits no later than their first birthday. Besides teeth cleanings and checkups for decay, your dentist has a number of measures like sealants or topical fluoride to protect at-risk teeth from disease.
Helping primary teeth survive to their full lifespan is an important goal in pediatric dentistry. It’s the best strategy for having healthy permanent teeth and a bright dental health future.

People have depended on dentures for generations—and they still do. That's because they work, both in restoring dental function and a smile marred by missing teeth.
But they have one major drawback related to bone health. That's because living bone has a life cycle: as older cells die, new ones form to take their place. The pressure generated when we chew stimulates this growth. But when this stimulus goes missing along with the teeth, the cell replacement rate slows and bone volume and density gradually diminishes.
Traditional dentures can't transmit this chewing pressure stimulus. And because they rest directly on the gum ridges, they can adversely affect the underlying bone and actually accelerate bone loss.
But implant technology potentially solves this bone loss problem with dentures by using implants rather than the gums to support them. It's a two-fold benefit: first, the implants relieve much of the irritation to the gums and bone caused by traditional dentures. Primarily, though, the implants themselves can slow or even stop continuing bone loss.
Most implants are made of titanium, not only because it's compatible with the body, but also because it has an affinity with bone. Over time bone cells grow on the titanium post imbedded in the jawbone. This process not only creates stability and durability, it can improve bone health.
In recent years dentists have incorporated implants with dentures to create two exciting treatment options. With one option, the dentist installs two or more implants in the jaw, to which a specially fitted removable denture can be attached. You would still have the ease of removing the denture for cleaning, while gaining greater stability and a reduced risk of bone loss.
The other option is a fixed denture (or bridge) attached permanently to implants. For this option, a patient's jawbone must be adequate and healthy enough to support at least four to six implants. A fixed denture is also often costlier and more complex than a removable denture, but it can feel more like real teeth. It also promotes better bone health too.
Although both options are more expensive than traditional dentures, they can pay dividends for long-term dental health. Implants could help you enjoy your new dentures and resulting smile for a long time to come.

In the battle against tooth decay, fluoride is an important weapon. Since the discovery of its dental health benefits a century ago, fluoride has been credited with saving countless teeth.
But over its history in dental care, this natural-occurring chemical has also had its share of controversy with concerns raised from time to time on potential health dangers. These run the gamut from “conspiracy theory” speculations to credible research like a 2006 National Research Council study that suggested a possible increased risk of bone fracture or cancer from over-consumption of fluoride.
Even so, there is actually little evidence or even record of incidence for such dire consequences. The only definitive health effect from fluoride found after decades of copious research is a condition called fluorosis, a permanent staining effect on the teeth. Fluorosis poses a cosmetic problem but does not harm the health of the teeth.
Moderation in fluoride use seems to be the key to gaining its health benefits while avoiding fluorosis. One influential fluoride researcher, Dr. Steven Levy, estimates 0.05-0.07 milligrams of fluoride per one kilogram of body weight (about a tenth the weight of a grain of salt for every two pounds) is sufficient to gain the optimum dental benefit from fluoride.
The real question then is whether your family’s current consumption of fluoride is within this range. That will depend on a number of factors, including whether your local water utility adds fluoride to your drinking water supply and how much. You may also be ingesting fluoride through processed foods, juices and even some bottled waters. And you can encounter fluoride in dental care including toothpastes and clinical treatments.
One way to moderate your family’s fluoride intake is to be sure all your family members are using the correct amount of fluoride toothpaste for their age while brushing. Infants need only a slight smear on the end of the brush, while older children can brush adequately with just a pea-sized amount. For other tips and advice, talk to your dentist about your family’s fluoride intake and how you might adjust it.
Even with the possibility of fluorosis, fluoride still provides an incredible benefit in preventing tooth decay. By understanding fluoride and keeping your intake within normal ranges you can maximize its benefit for healthier teeth and minimize the fluorosis risk.
